Position Summary: 
The RN-Case Manager works closely with the collaborative care team, assesses plans, intervenes, and reassesses an individual's needs to implement a plan of care in order to meet the needs of the individual(s). He/she assesses patient and family for clinical appropriateness of setting, and establishes plans for effective allocation of resource management for continuum of care. The Case Manager's responsibilities can include, but are not limited to concurrent reviews and the denial process, identifying discharge needs of patients, proactively interacting with various hospital departments to ensure diagnostic information is captured, and physician collaboration. Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Typically reports to a head of a department/unit.
Qualifications:
  • A certain degree of creativity and latitude is required.
  • Must be a registered nurse with 2-4 years of clinical experience within a hospital setting
  • Experience working with a hybrid medical record preferred.
